What the DOPL application
actually asks for
General Liability Certificate
A certificate of insurance (COI) showing your GL policy is active and meets DOPL minimum limits.
DOPL as Certificate Holder
The certificate must list the Division of Professional Licensing as the certificate holder — not just your name.
Workers Comp or Waiver
If you have employees, you need a workers comp policy. If you're sole proprietor with no employees, Utah offers a waiver path.
Entity Name Must Match
The business name on your insurance certificate must exactly match the entity name on your DOPL application. Mismatches cause delays.
